聲明 Declaration

本人謹以至誠，聲明由上述到期日至本聲明提交時間期間（「此期間」），受保車輛無涉及任何交通事故，同時本人不會就受保車輛於此期間發生之任何事故向澳門保險股份有限公司（「澳門保險」）索取保險賠償，承諾向澳門保險償付其倘因受保車輛於此期間發生之任何事故可能承擔的理賠損失及費用。本人保證上述聲明完整且真確無訛，並將作為本人重新投保汽車保險之根據。本人知悉並同意按澳門保險訂定之保單條款及細則，自本聲明書簽署日起重新投保。本人明白如上述資料錯誤或不詳實，本人及／或受保司機之保障有可能失效。 I hereby declare under utmost good faith that from the above policy expiry date to the time of submission of this declaration ("The Interim"), the insured vehicle has not been involved in any traffic incident. I will not make claim against Macau Insurance Co. Ltd. ("MIC") for any loss arising from any incidents(s) happened during The Interim. I will indemnify MIC for any loss, damage or expenses that may arise from any claims in The Interim. I guarantee that the information stated above is true and complete and will form the basis of this Reapplication for motor insurance. I understand and agree to reapply for a policy with effect from the date of this declaration, subject to the terms and conditions specified by MIC. I understand that if any information stated above is untrue or incomplete, the insurance cover for me and/or the insured driver may be invalidated.
